--Complete an enrollment form, including payment of a one-time enrollment fee of only $99. (As a current patient, if you have already received home whitening treatment from us in the past and have your whitening trays, your one-time enrollment fee is just $49. If you have received ZOOM treatment in our office, and have had your trays made by us, the enrollment fee is FREE!)
--Keep your cleaning and exam visits current with us.
--Provide us with at least 48 hours notice if you need to change your appointment.

--We'll provide you with a set of custom bleaching trays and whitening gel. Then, at your future cleaning appointments, you'll receive a free touch-up kit of bleaching gel (twice annually).
